These mice also failed to make antibodies after injections of 2 doses of hFIX in adjuvant, which is a more stringent test of tolerance. Similarly, HB mice that received a high or medium 1 109 TU kg ; dose of RV failed to develop anti-hFIX antibodies in response to 10 injections of hFIX without adjuvant. In contrast to the results with the high and medium doses of RV, some HB mice that received lower doses of RV as newborns developed antibodies after challenge with hFIX. For the low-dose 1 108 TU kg ; group, the frequency was statistically different from that in HB mice that did not receive gene transfer. Thus, although induction of tolerance was incomplete, it was still markedly reduced, and the antibody that developed was of low titer. For the very low dose 1 107 TU kg ; , the frequency of antibody formation with simple protein injection was harder to assess, given the small number of animals that survived, because of bleeding, but it was not statistically different from that in HB mice that did not receive gene transfer before protein challenge. All animals that received the very low RV dose developed antibodies after hFIX injection with adjuvant. We conclude that continuous expression of more than 14 ng mL hFIX starting shortly after birth results in tolerance to the administration of protein in adulthood. This is consistent with the observed tolerance in transgenic mice that express antigen at 10 8 though lower expression was insufficient to and entex.

Consequently, amplitude measurements have low sensitivity to detect central motor conduction failures due to the broad range of normal values. Since these characteristics are mostly due to varying desynchronization of the descending action potentials, causing different degrees of phase cancellation, we applied the recently developed triple stimulation technique TST ; to study corticospinal conduction to 489 abductor digiti minimi muscles of 271 unselected patients referred for possible corticospinal dysfunction. The TST allows resynchronization of the MEP, and thereby a quantification of the proportion of motor units activated by the transcranial stimulus. TST results were compared with those of conventional MEPs. In 212 of 489 sides, abnormal TST responses suggested conduction failure of various degrees. By contrast, conventional MEPs detected conduction failures in only 77 of 489 sides. The TST was therefore 2.75 times more sensitive than conventional MEPs in disclosing corticospinal conduction failures. When the results of the TST and conventional MEPs were combined, 225 sides were abnormal: 145 sides showed central conduction failure, 13 sides central conduction slowing and 67 sides both conduction failure and slowing. It is concluded that the TST is a valuable addition to the study of MEPs, since it improves detection and gives quantitative information on central conduction failure, an abnormality which appears to be much more frequent than conduction slowing. This new technique will be useful in following the natural course and the benefit of treatments in disorders affecting central motor conduction.
Cologic treatments that are most effective in dealing with the symptoms in this patient population. There is an excellent discussion of the role of the psychiatrist in the vexing question of withholding or withdrawing treatment. The authors of the chapter "Endocrine and Metabolic Disorders" begin by pointing out that the study of endocrine disorders may present the best opportunity to study the interaction of psyche and soma. Diabetes, understandably, gets the greatest attention. The brief review of the pathophysiology of Type 1 and Type 2 diabetes is useful. This is followed by a review of the literature exploring the possible role of psychiatric factors in the onset and course of diabetes. The literature on the role of "stress" is reviewed with the conclusion that the psychophysiology of glucose metabolism is still not clear. Of course, there is a review of the literature on the relationship of behavior compliance ; to diabetic control. The complex relationship between depression and diabetes is thoroughly reviewed. There is a particularly interesting discussion of the relationship between diabetes and eating disorders. The sections on the thyroid, parathyroid, and adrenal gland review the associated psychiatric disorders. In summary, a very thoughtful review of a fascinating topic. The chapter "Oncology, " written by two leading psycho-oncologists, is a state-of-the-art review of clinical psycho-oncology. The chapter is organized into several segments, starting with a section devoted to psychological factors affecting cancer risk and progression, followed by a review of psychiatric disorders in cancer patients depression, anxiety, mania, and delirium ; and a brief section on pain management the book also contains a chapter devoted to pain management ; . A total of 142 participants were enrolled into the study; 49 were randomized to the placebo group, and 93 were randomized to the treatment group. Of the 62 participants who did not complete the 1Zweek protocol, 13 complained of adverse reactions, 20 dropped out for personal reasons, 4 crossed-over to open label treatment at the 6 week visit, 8 were lost to follow-up, 5 violated study requirements, and 11 dropped out for unspecified reasons. There was only 1 death during the 12-week period, an individual in the placebo group who died due to an opportunistic infection.
