We found data to suggest that giving increased fluids to patients with respiratory infections may cause harm. To date there are no randomised controlled trials to provide definitive evidence, and these need to be done. Until we have this evidence, we should be cautious about universally recommending increased fluids to patients, especially those with infections of the lower respiratory tract.

All of the subjects had a fasting glucose level of more than 199mgt dL 11 mmol L ; when the study began. With intensive therapy, the fasting glucose level fell from an average of 239mg dL or 13.3 to 133mg dL or 7.0 mmol L, lead author Dr. Edmond A. Ryan and colleagues, from the University of Alberta in Edmonton, note. Moreover, this improvement was still present one year later.
Several novel chemotherapeutic agents moved forward following presentation of new data at ASCO. The lure of small molecule kinase inhibitors and monoclonal antibodies may lead one to believe that chemotherapy as an approach to the treatment of cancer will be replaced by these less toxic approaches. Certainly they have more tolerable toxicities in single agent studies than conventional chemotherapy but they are generally, with a few exceptions aside, less effective than chemotherapy. Most often they enhance the effect of chemotherapy by making cancer cells more susceptible to a cytotoxic approach. In other instances as with Erbitux in head and neck cancer, they enhance the effect of radiation therapy as a cancer treatment. Information was presented in a poster session on a subset of 44 patients who had de novo AML. The median age of the patients was 76 years. A 50% response rate was reported. Twenty patients had a complete response. Of these remissions 52% occurred in patients with intermediate cytogenetics and 47% occurred in patients with unfavorable cytogenetics. In the same de novo group a subgroup analysis of patients with poor risk factors, including unfavorable cytogenetics, poorer performance status ECOG 2 ; , cardiac, pulmonary and hepatic disease showed response occurred regardless of risk factors. Median survival in the the 22 de novo AML patients with complete remissions was 171 days compared to 47 days in the de novo AML patients who did not respond to therapy. Vion is justifiably enthusiastics about the prospects for cloretazine and views AML as underserved by contemporary therapy. Patients in the group studied here often receive only supportive care because the toxicity of current therapies outweighs the benefit. Nonhematologic toxicities appear to be low with chloretazine and myelosuppression thrombocytopenia and neutropenia ; is the dose limiting toxicity. Hematologists are very interested in cytotoxics that have myelosuppression as their major side effect. Dr. Judith Karp, Professor of Oncology and Medicine of the Leukemia Program at the Sidney Kimmel Comprehensive Cancer Center said "cloretazine has yielded provocative results in older adults with de novo AML. New agents agents are desperately needed if we are going to improve the outcome of patients for whom currently accepted approaches are clearly not adequate". Vion hopes that the activity and tolerability in this elderly group of patients with AML will be an acceptable path to FDA approval. In May 2002, legendary coach Elaine Michaelis retired from coaching after 40 years guiding the Cougar program. Karen Lamb, a BYU assistant the past eight years who also has been a college head coach for nine seasons, takes over a program that returns very little on-court experience from last year's 20-9 team. In 2002 BYU will be without the services of four key starters. Three time All-American Nina Puikkonen, fellow middle blocker Jackie Bundy and outside hitter Natalie Whittaker all graduated. In addition, senior rightside hitter Sunny Mahe, who was named the AVCA Sports Imports National Player of the Week and twice honored as the MWC Player of the Week in 2001, will redshirt the 2002 season while expecting her first child in November. Lamb takes over a Cougar team that enters the season without a returning All-American on the roster for the first time since 1995. Last year was the first season since 1989 that a Cougar did not receive All-America honors and only the fifth time BYU has not had an All-American since Lamb became BYU's first women's volleyball All-American in 1977. Just under two-thirds of patients in each arm had received oxaliplatinbased regimens 62 % and 64 % ; prior to the administration of irinotecan-based regimens. The response rate obtained with Erbitux irinotecan in the BOND study is higher than those observed with currently used second-line treatments, such as irinotecan 5-FU FA ; or oxaliplatin infusional 5-FU FA Table 10 ; .
